LAFAYETTE SCHOOL DISTRICT VOLUNTEER REQUIREMENTS

In the 24-25 school year, LAFSD will use "BE A MENTOR," as a way to support on-campus volunteering. Be a Mentor is a private online platform that will handle volunteer registration and screening.

First, volunteers must create a Be a Mentor account.

To complete the clearance process via the Be A Mentor portal, you will need:

1. State-Issued Driver’s License or State Identification
2. Tuberculosis (TB) Clearance:
   • Proof of TB clearance within the last four years is required. If you
     need TB clearance, consult your medical provider.
3. Live Scan Fingerprinting:
   • Download this Live Scan form and take it to your appointment.
   • If you completed Live Scan for LAFSD in the 2023-24 school year, repeating this step is not required as long as you create your Be a Mentor profile before August 31, 2024.
